The North West London Change Academy is now open for teams to submit their ideas on how to improve health and social care.
The Change Academy is an innovative leadership programme, designed to bring together teams from across health and social care at all levels in North West London. It has been created to support people who are leading or are interested in leading changes designed to embed integrated health and social care which align with our STP priorities. We are particularly interested in attracting multi-disciplinary teams who are working across current organisational boundaries.
How will the programmes work?
We are launching three programmes in 2017. The High Performing Care (HPC) programme and the Leading Transformation programme (LTP) are now open.
Teams applying for the HPC will require a senior leader to act as a sponsor. Sponsors will be given the opportunity to enrol on the LTP as the two programmes are intrinsically linked. You can find more details of these programmes on our website here.
The second phase will see the Commissioning for Outcomes programme launch in the coming weeks.
The programmes are a hands-on, intensive experience that will equip participants with the skills, expertise and approaches they need to tackle the most complex transformation challenges we face today. During the programme, participants will have the opportunity to come together away from their usual environment, have different conversations, and meet people who can share skills, expertise and insights from other systems and industries.
How do teams apply to join the HPC and LTP programmes?
We are seeking applications from existing teams rather than individuals, and are also looking to members of senior leadership teams within the NW London system to act as sponsors and nominate teams who they believe would most benefit from the programme.
Teams should be multidisciplinary and reflect our joint aspirations of health and social care integration. Teams should have identified a clear, citizen-focused transformation challenge, which is aligned to the priorities in the NW London STP, and which the Change Academy will help them to meet.
An example of a project which could apply for the support would be a project designing pathways of care for older people across primary, community care, hospitals and care homes involving social care, patients and carers.
